For project applications in which samples/data from the own collection are requested for secondary use (meaning that these samples will be used for a purpose NOT described in the study application),  FO.UBI.9 (for the current version see Forms), together with the necessary appendices, needs to be submitted to UBiLim. Approval of the Scientific Board and of an Ethical Comity is necessary as this application of the samples has not been approved. After approval of UBiLim, Scientific Board and Ethical Comity samples can be requested.
